As a clinic policy, patients that have not seen their GP in the past 3 years are inactivated for panel clean up purposes. Please ensure that you continue to touch base with your family doctor regularly to prevent this. It is good practice to see your GP about once per year for your annual periodic health screens or full physicals even if things are going well.
